What is CareLink?

CareLink is a health home program designed to coordinate care for patients who have complex chronic medical issues along with mental health disorders. CareLink increases access to health education, coordinates appointments and treatment with multiple providers.

What can CareLink help with?

CareLink coordinators work with clients to develop a plan that helps manage the physical and mental health services along with non-emergency medical transport. CareLink services assist with the reduction of emergency room visits and inpatient psychiatric admittances.

Who Qualifies?

  • Reside in Curry, Roosevelt, Quay, or De Baca County
  • Have a Serious and Persistent Mental Illness (SPMI) or Serious Emotional Disturbance (SED) Diagnosis
  • Have Turquoise Care
    • Blue Cross/Blue Shield
    • Molina Healthcare of New Mexico
    • Presbyterian Health Plan
    • United Health Care Community Plan
