High Fidelity Wraparound is a planning and facilitation process that follows a series of steps to help children and their families reach success, while remaining in their home communities. This process involves shared action from a committed group of family, friends, community resources, professionals and cross-system supports. Utilizing the family vision and story, the group creates a plan of care. Each team member contributes special resources and talents in order to meet the youth and family’s needs.
