New Patient Forms
What to Expect as a New Patient
After scheduling your first appointment, we’ll provide a link to complete your new patient forms. This allows us to review your information in advance and ensure your visit runs smoothly. Our team is always available to answer any questions and help you get started.
Appointments
We are happy to welcome new patients to our dental office anytime. At your convenience, we provide a flexible schedule to work with. If you are unable to make your appointment, call us with a 48-hour notice to reschedule.
Be sure to bring the following when you visit our practice for your initial visit:
- Insurance Information/Card
- Photo Identification
- Any medical records pertaining to your visit
If you did not receive paperwork in the mail, please arrive 15 minutes before your scheduled appointment to fill out the necessary paperwork.
Insurance
We accept most dental insurance plans. Our staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company.
Please know that you are responsible for your total obligation. We do not render our services on the basis that insurance companies will pay all of our fees. We base our treatment on what is best for you.
Payments
Payment is expected at the time services are rendered unless other arrangements have been made. This includes any copayments or deductibles (amount not covered by your insurance).
For your convenience, our office accepts cash, check, Visa, MasterCard, & Discover.
We realize that dentistry may be costly; therefore, we also have third-party financing options available:
frequently asked questions
Preventative Services
We generally recommend a check-up and professional cleaning every six months to help remove plaque and catch early signs of decay or gum disease.
During your visit we’ll perform an oral exam, remove plaque and tartar, take digital X-rays if needed, and apply protective measures like fluoride or sealants as appropriate.
Many dental plans cover routine cleanings, exams, and X-rays as part of their preventive benefits — though it’s always best to check your policy details.
Cosmetic Dentistry
Cosmetic options may include veneers, whitening, Invisalign® aligners, composite fillings, crowns, and other services that improve the appearance of your smile.
Yes. The goal is to blend with your natural teeth in color, shape, and alignment so your result is both aesthetic and natural-looking.
While cosmetic treatments focus on appearance, they can also enhance oral health by restoring damaged teeth or correcting alignment, which supports function and longevity.
Dental Implants
Patients who are missing one or more teeth, have healthy gums, sufficient jawbone, and are in good general health are typically good candidates.
The process generally involves placing a titanium post in the jawbone, allowing it to integrate, then attaching an abutment and artificial tooth or teeth.
With proper care — including good oral hygiene and regular check-ups — implants can last many years or even a lifetime.
Oral Surgery
This may include wisdom teeth removal, extractions, soft-tissue grafting, bone grafting, and other surgical treatments related to restorative or implant work.
We use local or appropriate sedation/anaesthesia so you’re comfortable during the procedure. Recovery times vary, but we’ll provide detailed instructions and support for healing.
Good preparation includes following pre-surgical instructions, arranging for post-op support, maintaining oral hygiene, and following all aftercare guidelines.
Dentures
Removable dentures rest on the gums and are taken out daily, while implant-supported dentures are anchored by implants for greater stability and comfort.
Yes — with proper fit and adjustment, dentures restore your ability to chew, speak, and smile confidently. Some adjustment time may be needed.
Clean them daily, remove them at night (if applicable), attend follow-up visits for adjustments, and contact us if they feel loose or uncomfortable.
Sedation Dentistry
Sedation dentistry uses medication to help you feel calm and relaxed during dental procedures. It’s offered in varying levels—such as nitrous oxide, oral conscious sedation, or IV sedation—depending on your needs and treatment.
When administered by trained professionals and preceded by a full medical review, sedation dentistry is considered safe. It’s often recommended for patients with dental anxiety, strong gag reflexes, or those needing longer treatments.
Before the appointment, you may be asked to share your medical history and adjust medications if needed. After treatment, you’ll need someone to drive you home, and it’s best to rest while the effects wear off.
